Your solicitor can give you the forms to complete and they can also help you fill them in.
Only solicitors that do publicly funded work will have the forms so look for solicitors firms that carry the quality mark sign. Also some Law Centres and Citizens Advice Bureaux can apply for Public Funding on your behalf.
