Free Living Will
Directions to medical staff about the circumstances under which you would not want to receive life saving treatment or treatment to artificially prolong your life. This document has to be signed by you and someone chosen by you to act as your "proxy" to ensure that your wishes are followed and to produce your living will when you are unable to.
